Verification of Monthly Benefit

There may be times when you need to provide verification of your monthly benefit amount to a third party. You may print an Income Verification letter by accessing your retiree account information through your Online Account, once you have registered to use the system.

You can call OPERS at 1-800-222-PERS (7377) to request an Income Verification letter be sent to you. The request should include your name and Social Security number. You may also send your request by mail to the following address:

If you would like OPERS to forward the information to a third party, you must also submit an Authorization for Release Account Information (LL-2) to OPERS. This is the only form of authorization for release that OPERS can accept.

If you would like OPERS to mail the letter to a third party, please be sure to provide the complete address where you would like the verification letter sent.

If you would like this information faxed to a third party, please include their fax number with your written request. Please allow five business days for us to process your request for an Income Verification letter.
