Medical Mutual Eye on Health Questionnaire and Wellness Resources

The Eye on Health Questionnaire is a new tool now available for OPERS participants in the Medical Mutual plan. By completing this brief questionnaire, you can learn more about your health and find out what steps you can take to become even healthier. Results will identify wellness resources that may fit your needs. These resources can help you set benchmarks, meet goals and lower the risk of and/or manage chronic conditions while providing ongoing coaching and support. The optional Eye on Health questionnaire is not required to participate in any of the programs listed below.

The Eye on Health Questionnaire is quick and easy! Visit and log in to your My Health Plan account. The questionnaire is listed under 'My Profile'.

Annual Physical and USPSTF-Recommended Preventive Services

Undergoing an annual physical helps you and your doctor identify potential health problems early, review any prescription and over-the-counter medications you are taking, and discuss ways you can achieve your optimal health.

To help in the prevention and/or early diagnosis of disease, preventive services are covered in full subject to frequency, age, and gender limitations based on the U.S. Preventive Services Task Force (USPSTF). The USPSTF was convened by the Public Health Service to rigorously evaluate clinical research in order to assess the merits of preventive measures, including screening tests, counseling, immunizations, and preventive medications.

Retiree Medical Account

Retirees enrolled in the OPERS Retiree Health Care Plan administered by Medical Mutual have access to a Retiree Medical Account (RMA) where funds are accumulated exclusively to pay for medical expenses not covered. These expenses include qualified medical, pharmacy, vision and dental costs. While the ability to add funds is no longer available, accumulated funds remain accessible in the RMA for a period of time.

To redeem funds, participants need to fill out a medical expense claim form.

After PayFlex processes the claim form, participants receive reimbursement through direct deposit by an automated electronic funds transfer (EFT) up to the amount of the RMA balance.

Advanced Care Planning

Equipped with the proper information, you can take control of your end-of-life planning and make choices regarding your finances, medical treatment preferences and care options. With the appropriate planning, you can feel secure in knowing your wishes and preferences will be supported and honored by someone you trust.

The end of life planning booklet, Choices, Living Well at the End of Life, will help you navigate through the advance directives process to determine your best course of action.
